Co-payments, co-insurances, and deductibles are expected before your visit with the provider and billed to a credit card on file.

Ouf-of-network insurance

If your insurance is “out-of-network”, we will provide a detailed receipt, called a “superbill” which can be submitted to your insurance company for possible reimbursement.

Please consult with your insurance company for any questions prior to your visit.

No insurance, no problem

If you are without insurance, full payment is due 24 hours before your visit. We accept all major credit cards, Cash app, Zelle, Venmo, PayPal, and HSA/FSA cards.

Private pay options

  • Initial Visit: $300 (60 minutes)
  • Follow-up visits: $175 (30 minutes)
  • INITIAL self-pay visits are charged full price ($300) of visit at the time of scheduling.

Other Fees

Patient initiated telephone calls:

  • 5 to 10 minutes   – $25
  • 11 to 20 minutes – $50
  • 21 to 30 minutes – $75

Scheduling or calls initiated by your provider are excluded from this charge. This is not usually billable to insurance.

Other fees not billable to insurance

  • Medical records preparation (paper copy) – $25 flat fee
  • Emotional Support Animal letter/additional forms – $100.00 flat fee
  • Disability forms/paperwork – $125.00 for each submission of paperwork

A valid credit/debit card is required to be on file when setting up an appointment for all patients excluding Medicare and Medicaid.
